Amoxil, generic name Amoxicillin, is a penicillin-based antibiotic. It is used to fight bacteria in the body and is prescribed for bacterial infections. Examples of such infections include bladder infections, E Coli, salmonella, gonorrhea, skin infections and ear infections. Amoxil might be used with Biaxin, or clarithromycin, to treat stomach ulcers resulting from a Helicobacter pylori infection. Prevacid, or lansoprazole, is a stomach acid reducer, also commonly prescribed in such cases.

Amoxil will not treat flu or the common cold. It is sometimes used before dental work or surgery to prevent infection. When treating conditions such as strep throat, it is vital to take the whole course of antibiotics, even if you feel better, to make sure the bacteria is totally eradicated.

Breast Cancer Metastatised to Liver After Surgery, Chemotherapy ...


Chan’s daughter told us that her mother’s loss of blood was due to the bleeding from her guts. The doctor in the private hospital told the family that nothing much could be done since the cancer had spread. Chan is a worrying-type of person and suffered from the following:

• She has pains if she walks for some distance.


• She is not able to sleep when she feels “heaty”.


• When she is asleep, she observes that her stomach moves and churns.


• Her stools are black in colour.

In December 2006 (about a year after taking Arimidex), Chan suffered a sudden onset of pains throughout her body besides having shortness of breast. Then, her guts bled leading to severe loss of blood. Are these problems related to the intake of Arimidex?

Among the many documented side effects of Arimidex are: hot flushes, mood disturbances, depression, fatigue, nausea and vomiting, muscle and bone pain, shortness of breath or coughing, fractures due to reduction of bone density, thromboembolic events, rash, swelling or water retention, loss of appetite, weight gain, constipation, diarrhea, vaginal bleeding and gastrointestinal disorder.

I also wake up four to five times every night.

Patient 3 wrote: I began taking Arimidex in late June and from the beginning had a constant headache, very slight so I didn't worry too much. Then the bone pain started and got worse very quickly. My oncologist wanted me to switch to "something else" right away.

Patient 4 wrote: My pains are definitely getting worse as each month goes by. I wake up feeling 30 years older. I am with pains pretty much all over the bones and joints from the hips down! I get depression too, very bad at times. Not sure if this is because I lack sleep due to painful nights, feeling sorry for myself or the effects of Arimidex.

Patient 5 wrote: Like you, at first I didn't notice any side effects. But for the last two to three 3 years I've experienced severe pain in my neck and back, specifically in the thorax region. It is truly debilitating at times. My doctor told me it was degenerative arthritis. But as I read your and other women's remarks who are on this treatment, I have to wonder if it is the Arimidex.

Patient 6 wrote: I just started taking this drug in June. At fist I was fine but recently I've been having pains. I wake up several times at night with pains in my arms and fingers.

Patient 7 wrote: I have been on this medication for seven months. I too have much joint pain -- pain during the night that wakes me up! I am wondering if it is worth all this pain -- there is no guarantee that in five years the breast cancer will not return!

Patient 8 wrote: I have been taking Arimidex for two years also. I feel bone pain, my hands hurt, my headaches have gotten worse, I am depressed, and my appetite is just not there any more. I wake up three hours after I go to bed. I feel tired and drained the rest of the day. My doctor thinks I am a hypochondriac. But these symptoms are real. My cousin had been taking this for four years and her intestine got infected from all the medicine. It started to eat away at it. She died two months after the removal of her intestine. Have you heard any thing about that?

The comments of these patients and the case presented by Chan are real. These are not fantasies of the patients’ mind. The side effects suffered by Chan are similarly felt by other patients. Chan had bleeding from her guts. Reading the comment of Patient 8, who wrote that her cousin who took the drug for four years, ended with an “eaten away” gut, one cannot help thinking if Arimidex is causing similar problem to Chan’s gut.

It is most distressful to see the CT scan of Chan’s liver. The metastasis is so extensive and widespread covering her entire liver. Over the decades looking at liver images, I cannot help feeling that the scan images of Chan’s liver is rare indeed.

The question that begs an answer: “Why is the liver metastasis so extensive?” Only a year ago (April 2006) Chan’s liver was said to be free of metastasis. Why does this happen so suddenly and so soon? It is understandable if there are only one or a few metastatic spots in her liver but to have numerous nodules at once is beyond comprehension. One is tempted to ask if this metastasis could be related to the treatment rather than the cancer itself.

The case of Chan further reinforces my resolve to say this to cancer patients: Cure for cancer requires a change of diet, lifestyle and full commitment on the part of patients to take matters into their own hands. Subjecting themselves to invasive treatments of modern medicine and taking of the so-called “FDA-approved drugs” may not be the only option for them. Look far and wide – there are other options available in the horizon. My experience shows that for some patients, taking the “alternative, holistic” path could prove to be a more humane and effective path for their cancers.

Grapefruit Juice: The Hidden Dangers Of Drug Interaction


Grapefruit juice is one of the healthiest foods around, right?

A cup of unsweetened white grapefruit juice has only 100 calories, no fat, more than 100% of the recommended daily amount of vitamin C, and it’s got a zingy taste that can really get you moving in the morning.

However, grapefruit juice (including the juice found in your morning grapefruit half) can interact with certain medications, leading to potentially serious consequences.

Which medications does grapefruit juice interact with?

Grapefruit juice can interact with many different drugs that people take to maintain their health. What Causes These Potentially Dangerous Interactions?

How can something as seemingly harmless as grapefruit juice affect the medications you take? It has to do with a special enzyme in your intestines and liver that help you absorb many oral drugs and then excrete them when you’re done with the drug.

When a physician prescribes a specific dose of drug (for example, one pill of 50 mg), she works on the assumption that given the size of your body, you will absorb the drug into your body at a certain rate and excrete it at a certain rate. Enzymes in your gastrointestinal (or GI) tract bring food and oral medications into your body. Grapefruit juice seems to affect both the rate of the drug coming into your body and how quickly it is removed. The end result can be an overdose of the drug) even if you’re taking the correct dosage for your size.

What Can I Do To Avoid Dangerous Drug Interactions?

If you are on medications that interact with grapefruit juices, avoid eating grapefruit or drinking grapefruit juice. Spacing out the drugs and the juice (for example, taking your medication at night and having grapefruit for breakfast) will NOT solve the problem; the grapefruit juice effect remains even after you’ve stopped having it. Colchicine Recall - Colchicine Side Effects & Colchicine Lawsuits


Drug manufacturer ApotheCure and the Food and Drug Administration enacted an immediate recall of compounded Injectable Colchicine .5mg/ml, 4ml vials, lot number 20070122@26 due to three recent deaths in Oregon. ApotheCure, a pharmacy based in Texas, recently mistakenly made this batch of an already dangerous drug 10 times stronger than the label stated. Colchicine is an extremely toxic nitrogen-based medication used by doctors to treat a number of conditions such as:

• Gout: Gout is caused when uric acid builds up in the cartilage of the tendons or joints throughout the body, which inflames the tissues and results in immense pain. Colchicine gout treatment side effects are extremely dangerous unless the drug is administered carefully


• Secondary amyloidosis(AA): This condition occurs when protein deposits build up in the body


• Scleroderma: When collagen builds up the skin or other organs it can cause scleroderma, which can be fatal in some cases


• Familial Mediterranean fever: This autoimmune disease affects people with ancestry primarily from around the Mediterranean Sea, and affects a wide number of systems including the chest, lungs, skin, and abdomen.

This recall is very similar to a recall that occurred in 2000 when Phyne Pharmaceuticals of Scottsdale, Arizona accidentally labeled their injectable colchicine .5 mg/mL, but it was actually the far more potent and potentially fatal 5 mg/mL.

Three people in Oregon have already died due mislabeled compounded colchicine. Compounding is a process that pharmacies sometimes use to create medications for people that have certain conditions that prevent them from using standard drugs. ApotheCure Inc. compounded colchicine for the Oregon patients, but made it too strong. Colchicine is already extremely dangerous under the right conditions, but when it is used incorrectly it can be almost immediately life-threatening. ApotheCure Inc. initiated a recall in order to prevent more fatalities from occurring.

Mechanism Of Action

NSAIDs work by suppressing the production of fatty acids called prostaglandins that cause inflammation and pain. They do this by blocking the action of an enzyme, cyclooxygenase (COX). This enzyme is responsible for converting precursor acids into prostaglandins.

In the periphery NSAIDs work by decreasing the sensitivity of the nociceptor to painful stimuli induced by heat, trauma, or inflammation. In the central nervous system, they are thought to function as antihyperalgesics and block the increased transmission of repetitive incoming signals to higher centers. In effect, they modulate perception of pain caused by repetitive stimulation from the periphery.

Differences Between NSAIDs

NSAIDs vary in their potency, duration of action, and the way in which they are eliminated from the body. Another important difference is their ability to cause ulcers and promote bleeding. The more an NSAID blocks Cox-1, the greater is its tendency to cause ulcers and promote bleeding.

Choice of NSAID for chronic and disabling inflammatory joint diseases like rheumatoid arthritis and osteoarthritis is governed by age, diagnosis, degree of severity, relative gastrointestinal safety, tolerability, and relative efficacy in the given clinical situation. It is a common misconception that all NSAIDs are therapeutically equally efficacious and any one of them could be used for the given indication. Use of multiple NSAIDs should be discouraged. An agent with comparatively less gastrointestinal (GI) side effects like ibuprofen and diclofenac should be preferred in place of indomethacin, piroxicam, or naproxen, which are more gastrotoxic. In conditions where diagnosis is uncertain, the medicine should be empirically chosen and given for a week or so and if the response is adequate it should be continued until side effects mandate its withdrawal. Ankylosing spondylitis responds better to a particular NSAID like indomethacin. It is probably related to its stronger inhibition of prostaglandin synthesis.

Aspirin is a unique NSAID, not only because of its many uses, but because it is the only NSAID that is able to inhibit the clotting of blood for a prolonged period (4 to 7 days). This prolonged effect of aspirin makes it an ideal drug for preventing the blood clots that cause heart attacks and strokes. Most other NSAIDs inhibit the clotting of blood for only a few hours.

The major NSAIDs of potency comparable to opioids are diclofenac and ketorolac. Moderate postoperative pain, for example, may be managed using these agents. The overall analgesic effect of 30 mg of ketorolac is equivalent to that of 6 to 12 mg of morphine. Efficacy has been demonstrated for postsurgical pain including oral, orthopedic, gynecologic, and abdominal procedures. Efficacy for acute musculoskeletal pain has also been shown. Ketorolac causes ulcers more frequently than any other NSAID and is, therefore, not used for more than five days.

Naproxen provides effective relief in acute traumatic injury and for acute pain associated with migraine, tension headache, postoperative pain, postpartum pain, pain consequent to various gynecologic procedures, and the pain of dysmenorrhea.

Possible Side Effects

NSAIDs are associated with a number of side effects. The two main adverse drug reactions, associated with NSAIDs relate to gastrointestinal effects and renal effects of the agents. These effects are dose-dependent, and in many cases severe enough to pose the risk of ulcer perforation, upper gastrointestinal bleeding, and death, limiting the use of NSAID therapy.


  • Gastrointestinal adverse reactions. The most common risk of NSAIDs is that they can cause ulcers and other problems in your esophagus, stomach, or small intestine. Common gastrointestinal side effects include: nausea, dyspepsia, vomiting, diarrhea, gastric ulceration/bleeding. Risk of ulceration increases with duration of therapy, and with higher doses. To help protect the stomach, NSAIDs should always be taken with food or directly after a meal.

  • High Blood Pressure and Kidney Damage. NSAIDs reduce the blood flow to the kidneys, which makes them work more slowly. When your kidneys are not working well, fluid builds up in your body. The more fluid in your bloodstream, the higher your blood pressure. If you take NSAIDs in high doses, the reduced blood flow can permanently damage your kidneys.

  • Allergic Reactions. NSAIDs can also cause extreme allergic reactions. People with asthma are at a higher risk for experiencing serious allergic reaction to NSAIDs. Many specialists recommend that people who have asthma stay away from any NSAID, especially if they have sinus problems or nasal polyps.




Use of aspirin in children and teenagers with chicken pox or influenza has been associated with the development of Reyes's syndrome. Therefore, aspirin and nonaspirin salicylates (e.g. salsalate) should not be used in children and teenagers with suspected or confirmed chicken pox or influenza.

Indomethacin, ketoprofen and piroxicam appear to have the highest prevalence of gastrointestinal adverse drug reactions, while ibuprofen (lower doses) and diclofenac appear to have lower rates.

Serious side effects are especially likely with one nonsteroidal anti-inflammatory drug, phenylbutazone. Patients of age 40 and over are especially at risk of side effects from this drug, and the likelihood of serious side effects increases with age.

Precautions And Contraindications

NSAIDs cannot be used in the following cases:


  • Allergy to aspirin or any NSAID

  • Aspirin should not be used under the age of 16 years

  • During pregnancy

  • During breast feeding

  • On blood thinning agents (anticoagulants)

  • Suffering from a defect of the blood clotting system (coagulation)

  • Active peptic ulcer




Cost

Numerous NSAIDs are available as generics: diclofenac, etodolac, fenoprofen, flurbiprofen, ibuprofen, indomethacin, ketoprofen, meclofenamate, naproxen, piroxicam, sulindac, and tolmetin. Only meloxicam (brand name: Mobic), nabumetone (Relafen), and oxaprozin (Daypro) are available by brand name only. Generic medications may be an equally effective and less expensive treatment option.


Conclusions

All NSAIDs are similarly effective. The choice of which NSAID to try first is usually empiric. If one doesn't provide adequate pain control, try switching to another. All NSAIDS when used chronically can be associated with the development of ulcers. Differences in adverse effects seem to exist between different NSAIDs. Follow with your doctor closely and watch for signs or symptoms of gastrointestinal bleeding such as stomach pain and blood in the stools. Some NSAIDs are available in extended-release formulations that require less frequent dosing.

Accutane, The Good and The Bad


Accutane is patented by the well-known drug manufacturer Roche Pharmaceuticals. In the United States, it goes by the brand name of Isotretinoin. In other countries, Accutane is known as Roaccutane. Some generic versions are also now available. These are known as Amnesteem, Sotret and Claravis.

Isotretinoin is derived from the A vitamin very much like Retin-A, but unlike Retin-A, isotretinoin is taken orally rather than used on the skin. Vitamin A has been proven to be very healthy for the skin but the unfortunate aspect is that the human body doesn't always assimilate vitamin A very well. In addition, if taken in large amounts, it can build up to dangerous levels. These toxins then can cause serious side effects. This fact has caused the medical professionals to exercise caution.

Isotretinoin on the other hand is very effective in battling acne because it is the only medication that works in many ways. It reduces the size of the sebaceous glands, decreases cellular accumulation, eliminates bacteria and lessons inflammation. Another positive attribute of isotretinoin is that it continues to exhibit productive results well after the individual stops using it. This drug may have a long lasting, positive effect on the skin and in the curing of the nasty symptoms associated with acne.

Accutane's most frequently experienced side effects usually involve the skin. An individual's skin may become very dry and quite irritated. The mucus membranes may also feel very much the same. One's lips and or eyes may also become extremely dry and irritated. Some other side effects might be muscle pain, joint pain, headaches, or the thinning of hair.

There are other more serious side effects, which do seem to disappear once the medication is no longer taken. These may include colitis, loss of night vision, or hepatitis.
